How to apply for criminal record in Vietnam?

Under the current laws on criminal records, a foreigner who has been living in Vietnam for more than 06 months may apply for the criminal records at the local Department of Justice where he/she lives.
The applicant must fill the standard forms and provide the copy of passport, visa and residence card (residence book or temporary residence certificate issued by the ward-level police authority where the applicant is living). The applicant must directly show the original document if the application dossier is not notarized.
The applicant may directly submit the application dossier or authorize another person for submission. In this case, it is required a notarized authorization document to the authorized person and the authorized person must bring his/her identity card when submitting the dossier. There is an exception when the authorized person is the father (mother), husband (wife) or child of the applicant, it does not require for notarized authorization document but the authorized person must bring a document to prove the relationship with the application (e.g. the certificate of birth, certificate of marriage or family residence book, etc...).
The application dossier must be prepared in 02 sets. The standard forms are Form No. 03/TP-LLTP (for direct submission or No. 04/TP-LLTP (for authorization case).
According to the Department of Justice of Hanoi, the assessment of application dossier will take 20 working days from the date of full and duly submission of dossier with a service fee of VND 200,000.
In the event that the foreigner who left Vietnam, requests for the criminal records for the residence time in Vietnam, he/she must apply the dossier at the National Center of Criminal Records under the Ministry of Justice.
